First Eucharist is two-year process for public school children, starting in first grade. Catholic school children may start in second grade. Please contact the director of Faith Formation if your public school child did not attend first grade Faith Formation. |

Confirmation is a two-year process for public school students, starting in ninth grade. Catholic school students are required to attend half of ninth grade and all of tenth grade. Please contact the director of Faith Formation if your child did not attend ninth grade Faith Formation.

|
Financial Assistance is available. See the dropdown list below the Family Address.

|
|
Bilingual Textbook (Spanish/English) is available for some classes. See the dropdown list in the Student section.

|
Health Problems: In the box labeled "Health Problems," list all info that might help us to meet the needs of your child, such as allergies, special learning needs, fears, medication, etc.

|
|
|
Special Circumstances: Put any special living arrangements or custody arrangements in the General Remarks box.
|
|
|
|
Emergency Contact: In the box labeled "Emergency Contact," list the name, relationship to the child, and phone number of the person(s) to call when the child is in a Faith Formation event.
|
|
|
|
Photos: Pictures taken during any St. James event may be used to bring attention to our parish community or the various aspects of our sacramental life, and may be used on the St. James website and social media. Give or refuse permission in the list labeled "Use Photos".

|
|
Walking home: Some families live within walking distance of St. James Church. Give or refuse permission for your child to walk home from Faith Formation classes in the list labeled "Walk Home".
